The different types of facelift can be explained well using the image of a tree:

The trunk represents the SMAS – the Superficial Musculo-Aponeurotic System. This connective tissue and muscular layer lies beneath the skin and forms the true structural framework of the face.

Tightening the SMAS is the foundation of every lasting facial rejuvenation. When this tissue is repositioned, the overlying skin follows the new tension entirely naturally – without tension, without mask-like effects. Aesthetic rejuvenation often begins in the mid-face: sunken cheeks, lost contours, and a tired appearance can all be effectively corrected in this way.

From this "trunk" the branches extend:

A neck lift, upper or lower eyelid tightening, a brow lift, or treatment of the jawline are all technically based on the same principle. They complement the SMAS facelift where further signs of ageing become visible. Many patients choose to address additional "branches" gradually, rather than opting for a comprehensive procedure all at once.

When a facelift makes sense – and when it does not

We recommend a facelift, for example, in cases of:

  • Loss of skin elasticity in the face and neck
  • Deep wrinkles that cannot be addressed with minimally invasive treatments
  • Sunken cheeks or drooping corners of the mouth
  • Significant excess skin in the neck area

We advise against a facelift in cases of certain medical conditions, poor physical health, during pregnancy and breastfeeding, or where expectations are unrealistic. A facelift cannot fundamentally alter facial anatomy; it can only reposition and rejuvenate existing structures.

Results

A facelift can significantly rejuvenate the face and neck, and accentuate contours (cheekbones, jawline). Depending on the technique, results last between 8 and 15 years. Importantly, the appearance remains natural and facial expression is preserved – no "mask effects", no "Hollywood facelift", no artificial tightening.

Risks and expectations

Every operation carries risks. These include post-operative bleeding, infection, swelling, nerve irritation, or prominent scar formation. Results may also differ from expectations. This is why a thorough assessment and evaluation before the procedure is just as important as the surgeon's experience.

Revision procedures and scars

Many patients come to us because they are dissatisfied following previous procedures. The reasons include visible scars, asymmetry, over- or under-correction, or unnatural-looking results. Dr. Karl Schuhmann regularly performs revision facelifts, including internationally. Corrective procedures require particular expertise and are considerably more demanding than a primary facelift.

A particular challenge lies in the management of scars: with extremely fine incision techniques, we are today able to place scars so that they are virtually invisible in everyday life. This is significant, because visible scars several millimetres wide are difficult to conceal and can cause dissatisfaction for years.

Costs

A frequently asked question concerns costs. These are highly individual and depend on the technique and the extent of the facelift.

  • Mini and One-Stitch facelifts are generally at the lower end of the price range.
  • More extensive procedures such as Deep Plane or Full Facelift are priced higher. Costs for a Full Facelift are approximately €15,500.
  • Revision procedures require greater effort and are priced accordingly.

SMAS Facelift | Classic Facelift | Full Facelift

In the classic SMAS lift (S.M.A.S. technique), the entire facial skin is detached from the SMAS connective tissue layer. SMAS stands for "Superficial Musculo-Aponeurotic System" – a thin yet fibre-rich connective tissue layer that lies beneath the skin and fatty tissue and is attached to the underlying muscles. During the facelift, the SMAS layer is mobilised, repositioned to its original location, and fixed in place. The subcutaneous tissue is tightened in this way, and any excess skin can be removed. The procedure concludes with a tension-free cosmetic skin closure. By tightening the SMAS, the soft tissues of the face are returned to their original position, restoring a youthful appearance without any mask-like effect. The SMAS facelift is also referred to as the classic facelift or full facelift.

Deep Plane Facelift

The Deep Plane Facelift is a well-established technique in facial rejuvenation. "Deep plane" refers to the deep layer and defines the surgical approach. In a Deep Plane Facelift, all key retaining ligaments and tissues in a deep layer are released while the superficial layers remain intact. Skin, fatty tissue, and muscles (SMAS) are lifted and tightened together. This allows particularly effective repositioning and tightening of the mid-face region. Compared to other facelift techniques, this is achieved with almost no tension, as the deep facial layers are displaced. The Deep Plane technique therefore enables very good and, above all, inconspicuous wound and scar healing. The Deep Plane Facelift consistently delivers particularly beautiful and natural-looking results. An overly tightened or stretched appearance is avoided.

Mid-Face Lift | Midface Facelift | Mid-Face Lift

The Mid-Face Lift, also known as a midface facelift or mid-face lift, is a facial rejuvenation procedure suitable for correcting pronounced nasolabial folds and drooping corners of the mouth. Through an incision beneath the lower eyelid, the cheek skin is released and the subcutaneous cheek tissue tightened. At the same time, under-eye bags can be removed and the lower eyelid tightened during this procedure.

Upper Facelift | Brow Lift | Forehead Lift

An upper facelift most commonly refers to a brow lift. The incision runs along the hairline from ear to ear. In addition to the skin, the forehead musculature is typically released as well. This technique also allows troublesome frown lines to be removed. An upper facelift can be performed using a classic open approach or endoscopically. Today, the endoscopic approach is preferred for brow lifting, as it leaves fewer scars.

Lower Facelift

The lower facelift differs in both the incision pattern and the nature and extent of the treatment. This depends on the degree of facial rejuvenation planned. Lower facelifts include:

  • the mini facelift – tightening of the skin in the cheek area without tightening the subcutaneous tissue
  • the mid-face lift – tightening of the skin with tightening of the deep subcutaneous tissue
  • the neck-cheek lift – tightening of the cheeks and neck area (neck lift)

Mini Facelift | Small Facelift | Minifacelift

In a mini facelift, the facial skin is tightened superficially and any excess skin is removed. The underlying issue of sagging facial contours is not addressed, as the subcutaneous tissue is not lifted in a mini facelift. The results of this type of facial tightening are short-lived, as skin ageing continues to progress.

ESP Facelift

In the ESP Lift (Extended Supraplatysmal Plane face lift), the skin, as in the SMAS lift, is separated from the SMAS connective tissue layer along with the fatty tissue. However, in this procedure significantly more skin in the facial area is detached, making it possible to address even pronounced crow's feet and deep nasolabial folds.

Composite Facelift

Composite lifting refers to a deep-plane lifting technique. In this procedure, the skin, cheek fat, and facial and neck musculature are simultaneously detached from the bone and tightened. The neck lift therefore forms part of a composite facelift. The composite facelift is based on the concept that the face ages as a dynamic, unified entity rather than as a group of isolated and independent parts.

Bio Facelift

Bio-lifting is not a specific lifting procedure as such. Rather, this term describes the additional use of a tissue adhesive (fibrin glue) during the operation to reattach the detached skin to the underlying tissue. Fibrin is a natural, endogenous clotting agent that stops bleeding and is intended to prevent bruising from occurring. In the hands of a qualified surgeon, complications are rare, though they cannot be entirely ruled out.

Cosmetic pre- and post-treatment of the skin can significantly enhance the outcome of aesthetic surgery and support the healing process. Incisions heal better when the tissue is well-perfused and free of congestion. Manual lymphatic drainage before the procedure helps to decongest the tissue. Removing dead skin cells using various peeling treatments is excellent preparation for the skin. Microdermabrasion, for example, gently removes the outermost skin layer, improving skin texture and stimulating cell renewal. Moisturising products, such as those based on stabilised hyaluronic acid, additionally nourish the skin, ensuring it is optimally prepared externally for the procedure.

Frequently asked questions

What should I be aware of before facial rejuvenation surgery?

14 days before the operation, you should not take any painkillers containing acetylsalicylic acid (e.g. aspirin). Its blood-thinning effect delays clotting and can cause post-operative bleeding. You should also avoid hormone-containing medications such as oestrogen preparations or the contraceptive pill. Reduce your consumption of nicotine and alcohol before the facelift to a minimum. If you regularly take medication or have allergies – for example to certain anaesthetic agents, medications, disinfectants, or plasters – please inform us without fail. You should also let us know if you are planning significant weight loss, as this may affect the result. If you smoke, you should ideally stop at least four weeks before your appointment to reduce the risk of wound healing complications. It is not necessary to shave the hair before the procedure. On the contrary: if you wear your hair very short, you should allow it to grow a little longer to help conceal the surgical scars. The procedure is usually performed on an inpatient basis. It can be carried out under general anaesthesia or, for smaller procedures, under local anaesthesia. Depending on the procedure, a hospital stay of one to five days should be expected.

What should I be aware of after facial rejuvenation?

After the procedure, you should plan for approximately one to two weeks off work. To support the reduction of swelling, you will wear a chin bandage for a few days. Please plan for one day in the clinic. You will be socially presentable after 2 weeks, as the swelling will largely have subsided by then.

What does "no Hollywood facelift" mean?

A "Hollywood facelift" refers to an unnaturally and excessively tightened face. Dr. Karl Schuhmann places great importance on natural-looking results: no mask-like facial expression, no artificial appearance.

What is a Super-extended Facelift?

The so-called Super-extended Facelift is an advanced technique used in complex cases – for example, where skin ageing is particularly pronounced.

What is a revision facelift?
