Many patients ask me whether the lower eyelids and under-eye bags can be treated without surgery. Unfortunately, a non-surgical lower eyelid lift and non-surgical removal of under-eye bags are not possible on a permanent basis.

The Simple Lower Eyelid Lift: What to Expect

In the simple lower eyelid lift, also known as a simple lower blepharoplasty, a precise incision is made along the lower eyelid crease. Excess skin and accumulated fatty tissue are removed, the skin overlying the muscle is tightened, and the wound is carefully closed with fine sutures. Thanks to this very delicate incision technique, the scar following lower eyelid correction is minimal to virtually invisible. Sutures are removed 7 days after the lower eyelid surgery. Recovery from the simple lower eyelid correction takes approximately one week for any swelling and minor bruising to resolve.

What Does the Extended Lower Eyelid Lift by
Dr. Karl Schuhmann

Involve?

The extended lower eyelid lift, performed by Dr. med. Karl Schuhmann, is a carefully planned and precise procedure aimed at preserving the youthfulness and radiance of the eye area. Various aspects of the treatment are taken into account to achieve optimal results.

Consultation and Analysis: Prior to the procedure, a thorough consultation is carried out by
Dr. Karl Schuhmann

. Individual wishes are discussed and a comprehensive analysis of the eye area is performed. Aspects such as under-eye bags, wrinkles, excess skin, and the condition of the lower eyelid tissue are assessed.

Treatment Planning: Based on the analysis,
Dr. Schuhmann

creates an individualised treatment plan. The extended lower eyelid lift may incorporate various techniques, including the removal of under-eye bags, tightening of the lower eyelid tissue, harmonisation of the eye area, and, where appropriate, repositioning of fatty tissue.

The Procedure: The extended lower eyelid lift is performed under local anaesthesia or, in some cases, under general anaesthesia.
Dr. Karl Schuhmann

makes precise incisions to open the skin and gain access to the areas to be treated. Under-eye bags are removed, excess skin is tightened, and fatty tissue is repositioned or reduced as required.

Aftercare and Recovery: Careful aftercare following the procedure is of paramount importance.
Dr. Schuhmann

and his team support you throughout the entire healing process. This includes wound care instructions, medication recommendations, and regular follow-up appointments to ensure that recovery proceeds optimally.

Long-Term Results: The results of the extended lower eyelid lift develop over time. Swelling and any bruising subside, and the final outcomes become visible. Patients benefit from a fresher appearance, a more harmonious eye area, and an overall more youthful look.

What Is a Non-Surgical Lower Eyelid Lift?

When people refer to a non-surgical lower eyelid lift, they mean the treatment of dark circles, deep tear troughs, and hollowing under the eyes, or skin tightening beneath the eyes.

Dark under-eye shadows and deep tear troughs can be minimised with
hyaluronic acid filler injections

. As
hyaluronic acid

breaks down over time, the
hyaluronic acid

treatment must be repeated approximately every 6–8 months to keep dark circles under the eyes at bay. A long-lasting correction of
dark circles

using the body's own material can be achieved with
autologous fat

injections. However,
autologous fat

treatment (
lipofilling

) requires good skin tone and muscle function in the face. Particularly good results with
autologous fat injections

for under-eye shadows and dark circles are achieved in younger and middle-aged patients. Skin tightening of eyelid wrinkles can also be achieved by means of a lower eyelid lift using a CO2 laser.

Causes of Puffy Eyes and Lower Eyelids

The causes of puffy eyes, swollen under-eye bags, crow's feet, and eyelid wrinkles vary. The skin around the eyes is very fine and delicate. With advancing age, the skin loses its elasticity, which also affects the tissue around the eyes. This loss of elasticity leads to a descent of the upper facial region. Fluid or fat can accumulate beneath the lower eyelids. These deposits of fat and fluid beneath the eye become visible as puffiness and under-eye bags.

Further Causes of Swollen Lower Eyelids, Under-Eye Bags, and Eyelid Wrinkles

  • Lack of sleep

  • Stress

  • Frequent sun exposure

  • Smoking

  • Poor diet

  • Genetic factors

  • Allergies

  • Dacryocystitis (tear sac inflammation)

Upper and Lower Eyelid Lift as a Combined Blepharoplasty

To tighten the eyelids, both an upper and lower eyelid lift can be performed in a single
blepharoplasty

procedure. Are you troubled not only by your under-eye bags or
Malar Bags

, but also by
hooded upper eyelids

that are impairing your field of vision? In that case, it is possible to combine an upper and lower eyelid lift in a single eye surgery.

Treatment methods

Standard Lower Eyelid Lift

Precise incision along the lower eyelid fold, removal of excess skin and fatty tissue, tightening of the skin. Recovery approximately 2 weeks.

Extended Lower Eyelid Lift

Additional tightening of the underlying musculature and tissue – comparable to a mid-face lift. Recovery approximately 3 weeks. Available exclusively with Dr. Schuhmann in Düsseldorf.

Non-surgical Treatment

Hyaluronic acid injections, autologous fat transfer, or CO2 laser. Not a permanent correction, but a good alternative for mild cases.

When is a lower eyelid lift advisable?

  • Pronounced under-eye bags

  • Lax lower eyelids

  • Malar Bags (cheek swellings)

  • Excess skin beneath the eyes

  • Tired, fatigued appearance

  • Dark circles under the eyes

Crow's feet are not a standard indication – botulinum toxin is more effective here.

Frequently asked questions

What Are the Requirements for a Lower Eyelid Lift?

The criteria for a lower eyelid lift vary. In general, however, all patients who wish to have their under-eye bags or Malar Bags removed are suitable candidates for lower eyelid surgery. Crow's feet and wrinkles under the eyes are not a classic indication for lower eyelid correction. Crow's feet and eyelid wrinkles can be treated very effectively with Botox injections.

What Is an Extended Lower Eyelid Lift?

In an extended lower eyelid lift, not only the excess skin beneath the eye is removed and tightened, but the underlying muscle and tissue are also released and tightened. The extended lower eyelid surgery is therefore more comparable to a mid-facelift than to a simple lower eyelid lift.

What Does the Lower Eyelid Correction Process Involve?

The process involved in a lower eyelid correction depends on the surgical method chosen. A lower eyelid correction can be performed using either a simple lower eyelid lift or an extended lower eyelid lift.

What Should I Bear in Mind Before My Lower Eyelid Lift?

Before your lower eyelid lift, you should stop smoking and avoid alcohol at least 2 weeks prior to surgery in order to improve the skin's oxygen supply and thereby accelerate wound healing. Pain medications such as aspirin, anti-rheumatic drugs, antipyretic medicines, and high-dose vitamin E should also be avoided during this period, as they affect blood clotting. Sleeping tablets should also be avoided where possible. Prepare a small supply of dressings and two to three soft cooling elements. A migraine mask with gel filling can also help reduce puffiness around the eye area after the procedure. On the day of surgery, please arrive without make-up, well rested, and bring a pair of dark sunglasses. Plan to take at least one week off work following surgery. After two to three weeks, any swelling from the procedure will generally no longer be noticeable in public. On the day of the lower eyelid surgery, please avoid cover-up cosmetics and creams. Clothing that does not need to be pulled over the head is recommended.

What Should I Bear in Mind After My Lower Eyelid Lift?

After a lower eyelid lift, you should cool the eyelid area sufficiently to minimise significant swelling under the eyes and haematomas. For the first 2 days following surgery, the best approach is to cool using the 20–40 method: cool for 20 minutes, then leave without cooling for 40 minutes. Eye drops and specialist eye creams can support the healing process. The upper body should be elevated to aid recovery from the lower eyelid lift. During the first week after surgery, it may be more comfortable to wear sunglasses due to haematomas, bruising, and swelling under the eyes. Sunglasses will also protect your eyes from sun exposure. After a simple lower eyelid lift you will generally feel presentable in public again after approximately 2 weeks; after an extended lower eyelid lift, this is approximately 3 weeks. You should refrain from sport for 2–3 weeks. Anticoagulant medications should continue to be avoided following lower eyelid surgery.

What Are the Risks of a Lower Eyelid Lift?

The risks associated with a lower eyelid lift are very low. Lower eyelid correction is considered a very low-risk cosmetic procedure. Swelling beneath the eye and haematomas resolve within a few days. Despite being a minor procedure, lower eyelid tightening has a significant impact, resulting in a more alert and radiant gaze.

How Long Is the Recovery Period After a Lower Eyelid Lift?

How long does recovery take after a lower eyelid lift? The recovery period following a simple lower eyelid lift is approximately two weeks. For the extended lower eyelid lift, recovery may take up to 3 weeks.

How Long Do the Results of Lower Eyelid Correction Last?

Lower eyelid correction is a highly effective routine procedure that restores freshness and youthfulness to your face and eyes. Should under-eye bags, fluid retention beneath the eyes, or swollen lower eyelids recur with advancing age, lower eyelid correction can be repeated after approximately 10 years.

What Can Be Done About Under-Eye Bags?

Under-eye bags are visible swellings beneath the eye. They are most commonly hereditary. The connective tissue weakens and the fatty tissue beneath the eyes descends. What can be done about under-eye bags? Sufficient sleep and a healthy, balanced diet can help. Cold packs (gel pads) and cooled tea bags – black tea works best – can help reduce the puffiness. For pronounced under-eye bags, surgical lower eyelid correction is often the only effective option.

What to Do About Dark Circles – Causes of Dark Circles

Dark circles can have various causes, such as tiredness, insufficient sleep, a hereditary predisposition, or excessive alcohol and tobacco consumption. In most cases, dark circles do not indicate an underlying medical condition – they are primarily a cosmetic concern. The skin beneath the eyes is very thin, which means that blood vessels can show through, causing dark shadows under the eyes. A healthy lifestyle, adequate sleep, proper hydration, and regular exercise can help prevent dark circles and reduce lower eyelid puffiness. If this is not sufficient, various treatment options are available to address dark circles. Dark shadows under the eyes can be treated very effectively with hyaluronic acid fillers or a lower eyelid lift.
