What the Bullhorn Lift changes
The incision runs in an arc directly below the nasal base – hence the name "Bullhorn". A narrow strip of skin is removed directly under the nose. The skin is then sutured so that the upper lip rotates slightly upward. This shortening mechanically pulls the upper lip upward, causing it to tilt slightly outward. More of the red lip surface becomes visible and the lip appears fuller. This fundamentally distinguishes the Bullhorn Lift from hyaluronic acid treatments.
Who is this treatment suitable for?
Typical situations of our patients:
- large distance between nose and upper lip
- upper lip "disappears" when speaking or smiling
- little visible red lip surface
- insufficient results after filler treatments
- age-related lengthening of the upper lip
With increasing age, the distance between nose and lip grows longer. At the same time the lip rotates inward and appears narrower. We reverse this effect with the Bullhorn Lift.
Results and effect
The effect is visible immediately after the procedure: the upper lip appears lifted and the contour more defined. Proportions look more balanced. Unlike filler treatments, we add no extra volume. The change is achieved through the position of the lip, making the result appear more natural. After complete healing, once swelling has subsided, a stable result is achieved.
Advantages and limitations
Advantages
- permanent change of lip position
- no repeated treatments necessary
- clear contours without volume increase
- visible teeth when smiling
Limitations
- surgical procedure with scar formation (usually well hidden at the nasal base)
- not suitable for very short upper lips
- no solution for volume loss in the entire lip area
Combinations and complementary treatments
Depending on the individual situation, combinations are possible and sensible – especially when both lip position and skin quality or lip structure need to be improved.
Possible combinations:
- Autologous fat (microfat/nanofat): improvement of lip and skin quality
- Biostimulators: support of tissue structure
- Skinbooster: hydration and fine structural improvement
Risks and healing
As with all surgical procedures, swollen areas or temporary tension sensations are possible. The scar runs along the natural nasal fold and usually heals inconspicuously. In the first few days you should avoid physical exertion. We remove the sutures after approximately one week.
Costs
The costs depend on the individual findings and the scope of treatment. Transparent information is provided during a personal consultation at artethic Dr. Karl Schuhmann & Colleagues in Düsseldorf.

Frequently asked questions
How long does the result of a Bullhorn Lift last?
The result is permanent. The removed skin does not grow back. However, skin ageing continues, meaning tissue may change again over the years.
Can a Bullhorn Lift be combined with other lip treatments?
Yes, in certain cases a combination can make sense. If skin quality or fine structures also need improvement, complementary treatments such as autologous fat or regenerative procedures can be used. Which combination is appropriate depends on the individual findings.
Where is the scar from a Bullhorn Lift?
The scar runs directly below the nasal base along the natural skin border. In most cases it heals inconspicuously, as it lies in the transition between nose and upper lip. In the first weeks it may still be slightly visible, but it fades significantly over time. Individual scar healing depends on skin type and predisposition.
Does a Bullhorn Lift change the shape of the nose?
The shape of the nose is not changed. Only the distance to the upper lip is shortened. This may slightly alter the perception of the nasal base due to the adjusted proportions.
