In a breast reduction, an overly large breast is reshaped to be in proportion with a woman's body. Often it is not only the shape of the breast but also its size with which a woman is dissatisfied. The psychological burden of an overly large breast should not be underestimated. Sporting activities, beach holidays, or visits to the sauna are frequently avoided out of embarrassment or because of neck and back pain. The goal of a breast reduction – a beautiful breast that suits your body
Our goal is to create an anatomically beautiful breast that is in harmony with your body. Of the countless surgical techniques currently available for breast reduction, 10 methods are most commonly used. Each surgeon has their own individual approach to reducing the breast. The differences in breast surgery lie primarily in the incision pattern and the surgical technique. Many criteria – such as breast size, chest shape, and the desired cup size – ultimately determine which scar-minimising technique is used for the breast reduction.
Surgical techniques for breast reduction

"Central pedicle" technique
According to Lejour
This technique is suitable when only a small amount of breast glandular tissue needs to be removed. The nipple is incised circumferentially so that it can be separated from the breast gland. This small incision allows the removal of excess skin and glandular tissue. Unfortunately, this method leaves slight wrinkling around the areola. Its advantage lies in minimal scarring. A (periareolar) lift in combination with this type of reduction permits only modest overall reductions.
Other breast reduction techniques differ primarily in the resulting scar pattern (T-scar, L-scar) and in the location of the glandular segment removed and the segment that remains attached to the nipple. The T-scar technique includes the McKissock or Robbins procedure, where an inferior glandular pedicle carries the nipple.
With this breast reduction technique, the areola and the skin areas to be removed are incised, and the entire breast gland is then fully exposed.
Unlike the other techniques, the gland remains fully attached to the pectoral muscle. This preserves the blood vessels that pass through the muscle into the breast gland. The nipple also remains on the breast gland in this technique, so that its vessels and nerves can be protected. Sensation is preserved. A large volume of breast tissue is reduced in the upper, outer portion, while little is reduced in the lower portion. Depending on the individual body shape, it is possible to adapt the tissue removal and shape the breast accordingly.
The distinctive feature of this technique is the "internal suspension plasty" – the so-called "internal bra". Once the skin has been separated from the breast gland, a cup-shaped bra is fashioned from the subcutaneous layer of the excess skin. After shaping and modelling the breast gland, its lower portion is enveloped by this cup-shaped bra. This support prevents the breast from sagging too quickly and ensures that the shape is maintained for a long time.
Because there is less tension on the scars, they tend to remain narrower. A further advantage is that the ability to breastfeed is preserved with this technique. The disadvantage lies in the length of the scar. Nevertheless, the benefits of this approach – such as shape and longevity – often outweigh the drawbacks. Very few breast surgeons currently perform this technique, as it requires a high level of skill and experience to carry out successfully.
A standard reduction technique is that of the Belgian surgeon Dr Madeleine Lejour. The incision is made around the areola and extended in an arc down to the inframammary fold. The skin excision pattern resembles a keyhole.
First, the upper skin layer is removed without damaging the blood vessels essential for nipple perfusion. The breast gland can then be lifted away from the pectoral muscle, starting from the inframammary fold. The volume to be removed is excised in a cone shape from the centre of the gland, leaving an inner and an outer glandular pillar. These two pillars are sutured together to form the new breast. After lifting and fixing the nipple in its new position, the skin envelope is tightened and sutured. The vertical skin scar gathers slightly, owing to the scar-minimising special suturing technique.
During the first few months after surgery, this scar gradually smooths out. However, a small excess of skin (a "dog ear") not infrequently remains at the lower pole of the scar, at the level of the inframammary fold. This can be corrected under local anaesthesia six months after surgery.
What should I consider before a breast reduction?
Approximately two weeks before a breast reduction, you should avoid pain medications containing acetylsalicylic acid, such as aspirin, as their blood-thinning effect can cause unwanted post-operative bleeding. Nicotine and alcohol should be significantly reduced prior to surgery. Please also inform me directly during the initial consultation about any allergies or clotting disorders.
Before a breast reduction, you should in any case have a mammogram carried out. Ideally, plan to take two weeks' leave for your recovery from the breast reduction.
Prior to every breast reduction, we determine precisely what the future shape of the breast should look like and how much glandular tissue is to be removed. In practice, this means: with the patient standing, we draw the incision lines on the skin using a marking pen. The distances to the sternum, the clavicle, or the future nipple position are carefully measured and recorded. I perform all breast operations under general anaesthesia.
What should I bear in mind after a breast reduction?
As with any surgical procedure, breast reduction can be associated with bruising, swelling, and discomfort. Depending on the surgical technique, a loss of nipple sensation and loss of the ability to breastfeed are also possible. This is the case when the nipple must be detached and subsequently re-grafted due to the size of the breast. There is no single standard breast reduction; rather, there is a wide range of surgical techniques that must be tailored individually.
Depending on the breast reduction technique chosen, you should expect a hospital stay of 3–8 days. Immediately after the breast reduction, the breast area is secured for several days with a firmly applied tape dressing. The drains, which allow wound fluid to drain away, are removed 1–2 days after surgery. The feeling of tightness in the breasts is relieved with pain medication. Bruising resolves within a few days, and the swollen areas subside over the course of several weeks. Sutures are removed after 14 days.
After the breast reduction, physical exertion (sport, physically demanding household or occupational activities) should be avoided for 6 weeks. Once the dressing has been removed, a well-fitting, non-restrictive support bra/sports bra must be worn both day and night for 6 weeks to support the healing process. Please bear in mind that during this period you should sleep on your back and handle your breasts with great care. Avoid underwired push-up bras for a further 3 months, and protect the surgical scars from intense sun exposure for at least 6 months.
