What are the causes of gynaecomastia | male breast?

A particularly prominent male breast may involve either an enlargement of the breast gland (which is also present in men) – known as true gynaecomastia – or breast development caused purely by an accumulation of fatty tissue, referred to as pseudo-gynaecomastia. In most cases, the cause of "true" gynaecomastia is a high oestrogen level in the body, for example as a result of hormone-containing medications.

Whilst these signs can resolve spontaneously in adolescents during puberty, they can become a persistent and distressing condition in older patients. In addition, severe obesity, long-standing liver and kidney disease, and excessive consumption of food, alcohol, and marijuana can be responsible for an extraordinary degree of breast enlargement.

Pre-operative assessments take place on the day before the operation. Immediately before the procedure, the incision lines are marked directly on the chest area. The surgical procedure takes approximately 1 – 1.5 hours, though more complex corrections may take longer. In some cases the procedure is performed on an outpatient basis; in others, a hospital stay of 1–2 days is required.

Pure liposuction requires small incisions in the chest through which appropriate cannulas are introduced to suction out the fatty tissue. Excess glandular breast tissue is removed via an incision around the lower border of the areola and, where necessary, a further incision beneath the axilla. Where the skin is already significantly stretched, a skin tightening procedure is performed.

The procedure leaves scars which, through the use of the finest sutures and a special subcutaneous suturing technique, will be barely noticeable. Gynaecomastia surgery carries low risks in experienced hands; however, complications are possible during and after any surgical procedure, even if rare.

What happens after gynaecomastia surgery?

Immediately after gynaecomastia surgery, the chest area is supported with a compression band or compression garment for two weeks. Drains are removed one day after the procedure. Any sensation of tightness is relieved with pain medication. Bruising resolves within a few days, and swelling subsides over several weeks. Sutures are removed 14 days after the operation.

Following the procedure, physical exertion (sport and physically demanding occupational activities) must be strictly avoided for approximately 3 weeks. The compression garment should be worn both day and night for 1–2 weeks to support the healing process. After that, it is sufficient to wear the compression garment only at night for a further few weeks. Protect the surgical scars from direct sun exposure for at least 6 months. This will positively support the healing process, and after approximately 3–4 months you should be satisfied with the overall result.

Is gynaecomastia dangerous?

Gynaecomastia is not a dangerous condition. However, if you are significantly affected by the breast enlargement, you should consider gynaecomastia surgery. Surgery to address the male breast may also be appropriate in cases of breast pain or restricted movement.

Is gynaecomastia surgery covered by private health insurance?

Gynaecomastia surgery is generally considered a purely aesthetic procedure. Even though the degree of distress experienced by affected men may be considerable, male breast removal is in most cases not a medically necessary procedure. Only where verifiable physical symptoms attributable to gynaecomastia are present can the costs of breast reduction in men be covered in full or in part by private health insurance.
