What surgical techniques are available for breast augmentation?


In breast augmentation with implants, the surgeon has several options for placing the silicone implant:

Implant placement


Breast implant above the pectoral muscle (subglandular)


In patients with well-developed glandular tissue, the implant may be placed above the pectoral muscle. This technique is particularly appropriate for women seeking a more pronounced increase in volume. Where sufficient native tissue is present, straightforward subglandular placement may be the right choice.


This approach offers advantages including a faster recovery with minimal discomfort and a very natural-looking breast contour. The technique enables a quick return to normal activities and minimal post-operative pain, and produces a very natural appearance.


Some surgeons prefer to place implants above the pectoral muscle in order to avoid distortion caused by muscle contraction in athletically active women.


Implants placed above the muscle carry the disadvantage that any distortion tends to be more visible externally. The edges of the implants are more easily seen and felt. Slim patients often find this unsatisfactory after augmentation, as the natural feel of the breast is not optimal.


Breast implants beneath the pectoral muscle (submuscular)


Implants are routinely placed beneath the pectoral muscle. This is intended to reduce the risk of complications and also facilitates breast cancer screening. The risk of
capsular contracture

is also lower with this approach.


Capsular contracture

refers to the formation of a hardened fibrous capsule around the implant. This problem occasionally arises following breast augmentation with implants and may require further corrective surgery. In addition, an implant placed beneath the pectoral muscle is less easily palpable.


Placing the implant beneath the pectoral muscle is preferable in slim women, as this makes it less visible and less palpable. Because the pectoral muscle is not very elastic, the submuscular technique is generally preferred when smaller implants are being used.


Dual-plane technique – subpectoral implant placement


With subpectoral implant placement, the pectoralis major muscle is draped over the greater part of the implant. The lower third remains uncovered and is – as in the subglandular technique – covered only by the breast glandular and adipose tissue. This surgical technique is also referred to as the dual-plane technique.


This surgical method combines the advantages of both the submuscular and subglandular approaches to implant-based breast augmentation. The technique enables the surgeon to position the implants securely, resulting in a natural-looking breast with the desired shape.


What is breast augmentation with an internal bra?


Breast augmentation with an internal bra is a submuscular technique that effectively prevents implant displacement. In implant-based breast augmentation, placement of the silicone implant behind the pectoral muscle is frequently chosen.


In submuscular breast augmentation, the surgeon fashions a pocket from muscle flaps to accommodate the implant. The implant is then usually sutured in place, creating a particularly secure and stable breast shape that resists movement.


This surgical technique is therefore also referred to as the "internal bra technique." Its advantages include minimal implant edge visibility owing to optimal implant coverage. Capsular contracture also occurs somewhat less frequently than with the subglandular technique.

What should I be aware of before breast augmentation?


Approximately 2 weeks before surgery, you should avoid analgesics containing acetylsalicylic acid, such as aspirin, as their blood-thinning effect can cause unwanted post-operative bleeding. You should significantly reduce your nicotine and alcohol consumption in the lead-up to surgery. Please inform me directly at the initial consultation of any allergies or coagulation disorders. A mammogram should be obtained prior to breast augmentation in every case. The inpatient hospital stay is approximately 1–2 days. We recommend planning for 2 weeks off work and regular activities to allow for recovery.


How does breast augmentation surgery proceed?


Pre-operative examinations take place on the day before surgery. In straightforward cases, implant-based breast augmentation takes less than 1 hour and is performed under general anaesthesia. Immediately before the procedure, I mark the incision lines directly on the breast area and prepare you for surgery. In order to introduce the implants, a discreet incision is made and an implant pocket is carefully prepared.


Depending on the individual anatomy of the breasts and the patient's preference, the implants can be introduced via the axilla (transaxillary), the inframammary fold (inframammary), or the areola (periareolar). The implant pocket can be created above the pectoral muscle (subglandular) or beneath it (submuscular). The lowest risk of
capsular contracture

is associated with submuscular placement. The periareolar approach is highly advantageous, as scars at the areolar border are barely perceptible and optimal implant positioning can be achieved. Where the breast is ptotic, a
breast lift

must be performed in addition.


What should I be aware of after breast augmentation?


Immediately after surgery, the breast area is secured with a firmly applied tape dressing for several days. Drains are removed 1–2 days post-operatively. Any feeling of tightness in the breasts is managed with analgesics. Bruising resolves within a few days, and swelling subsides over several weeks. Sutures are removed after 14 days.


Following breast augmentation, physical exertion (sport, physically demanding domestic and professional activities) must be completely avoided for approximately 4 weeks. Once the dressing has been removed, a well-fitting, non-constricting support bra or sports bra must be worn day and night for 6 weeks to support the healing process.


Please bear in mind that during this period you should sleep on your back and treat your breasts with great care. Avoid underwired or push-up bras for a further 3 months, and protect the surgical scars from intense sun exposure for at least 6 months. Following these guidelines will have a positive influence on the healing process and allow you to be satisfied with the overall result after several months.


Breast augmentation is in most cases permanent; however, manufacturers recommend replacement after approximately 10 years. In our view, this is only necessary if problems arise. For your own peace of mind, I recommend attending regular preventive check-ups. Health insurance funds very rarely cover the cost of breast augmentation, as it is in most cases an aesthetic rather than a primarily medical procedure.


What should I do in the case of
capsular contracture

?


Capsular contracture

is a hardening of the tissue surrounding the breast implant. A fibrous, sometimes painful capsule forms because the body recognises the implant as a foreign body and encases it. This foreign body reaction can lead to distortion of the breast. As long as the capsule remains soft, this is a normal process. It is only when the capsule hardens and distorts that symptoms arise which require surgical treatment.


Capsular contracture occurs in fewer than 5% of implant-based breast augmentations. However, if you are affected by

What is the DeBreast method?

The use of drains in breast augmentation has, however, long been a contentious topic of discussion in Germany. Drawing on many years of experience, it can be said that wound drains do indeed serve a purpose. A wound that was dry during surgery may well produce wound secretion the following day. If this is removed via the drain, haematoma formation can be prevented. If it is not, the processes of haematoma breakdown and resorption may lead to

. For this reason, omitting the drain after breast surgery is not always advisable.


Breast implant exchange


After a certain period of time, breast implants need to be replaced. Breast implant exchange most commonly takes place after 10–15 years or when complications arise.


What is a bottoming-out breast procedure?


A possible complication following breast augmentation is known as bottoming out. This term refers to the downward displacement of a breast implant below the inframammary fold. The nipple then no longer sits in the centre of the breast but points upward.


The displacement of the breast implant does not necessarily cause pain. However, the following signs or symptoms may indicate a change in the position of the implant:
The distance between the nipple and the inframammary fold increases.
The lower aspect of your breasts bulges downward.
Your nipples appear to be gradually moving upward from their central position.

A fibrous, sometimes painful capsule forms because the body recognises the breast implant as a foreign object and encases it in a shell. This foreign body reaction can lead to a distortion of the breast. As long as the capsule remains soft, this is a normal process. Only when the capsule hardens and deforms can it cause symptoms that require surgical treatment. Capsular contracture occurs in fewer than 5% of breast augmentations with implants. Should you be affected by capsular contracture following breast augmentation, this can only be corrected by a further operation, during which the old breast implant is removed and replaced with a new one.

What is the DeBreast method?


The DeBreast method is a technique designed to make breast augmentation as near to pain-free as possible and to promote a faster recovery following breast surgery. The following factors play a key role: – The correct, tissue-sparing surgical technique – Specialist instruments – Omission of a drain. The use of drains in breast augmentation has, however, long been a contentious topic of debate in Germany. From my many years of experience, I can say that wound drains do serve a genuine purpose. A wound that was dry during surgery may well produce wound secretions the following day. If this is drained away, a haematoma can be prevented. If it is not, the processes involved in the formation and breakdown of a haematoma can lead to capsular contracture. For this reason, omitting the drain after breast surgery is not always advisable.

Breast Implant Replacement


After a certain period of time, breast implants need to be replaced. Implant replacement most commonly takes place after 10–15 years, or when complications arise.

What is bottoming out in breast surgery?


One possible complication of breast augmentation is so-called "bottoming out." This term describes the descent of the breast implant below the inframammary fold. The nipple then no longer sits at the centre of the breast but points upwards. The slippage of the breast implant does not necessarily cause pain. However, the following signs or symptoms may indicate a change in the position of the implant: – The distance between the nipple and the inframammary fold increases. – The lower portion of your breasts bulges downwards. – Your nipples appear to gradually shift from the centre upwards.
