What is an extended One-Stitch facelift?
The One-Stitch facelift is a minimally invasive, outpatient variant of the SMAS facelift. Through a tiny incision we lift the skin and the underlying tissue without tension. Excess skin is removed and sunken areas are "filled" with microfat or nanofat.
Compared with injectables the tightening is considerably stronger; compared with a classic facelift it is more subtle, gentler and less intensive at the same time. The result looks very natural.
Why "extended"? Tightening plus autologous fat
With the extended One-Stitch facelift we inject autologous fat into thin or sunken skin areas. The reason: when volume is missing, tightening alone is not enough.
- The jawline (chin contour) and outer face are tightened.
- The midface is smoothed with micro- or nanofat.
- Volume and skin quality improve over the long term.
The gentle tightening and slight "lifting" of the tissue are visible immediately after the treatment. The full effect of the nanofat (collagen build-up in the skin) develops over several weeks.
Not to be confused
Not a thread lift: The extended One-Stitch lift is not a thread lift but a minimally invasive tightening of the SMAS layer.
Not a mini facelift: With a mini facelift only the skin is tightened, which then usually sags again after a short time, often just a few weeks. With the One-Stitch facelift we reposition the underlying tissue without tension. The effect is therefore more stable and the result more natural.
Who is the extended One-Stitch facelift suitable for?
This small procedure can be an option even at a younger age, after weight loss, for very slim people and with the first signs of skin ageing:
- Between approx. 35 and 55 years of age
- Beginning skin laxity
- After significant weight loss or weight-loss injections such as Ozempic®, Wegovy® or Mounjaro®
- Volume deficiency with a slim build
Autologous fat – more than just volume
Autologous fat is one of the body's own substances and is usually well tolerated by sensitive skin. Unlike artificial fillers, it is also ideal for thin skin areas such as the eye or mouth region. Autologous fat contains stem cells and growth factors that:
- improve the skin structure
- stimulate collagen production
- slow down the ageing process

When do we recommend the extended One-Stitch facelift?
The combination treatment is suitable above all for patients with beginning to moderate skin laxity, often between 35 and 55 years of age. Ideal when a loss of contour (sagging chin-jaw line) and a lack of volume (sunken cheeks or dark circles) occur together. Slim people after significant weight loss, for example after weight-loss injections, can also benefit from the procedure. What is decisive is always the current state of your skin and the desired result.
Why combine it with autologous fat?
Tightening alone can lead to a flat or 'hard' result when there is volume loss. The combination with autologous fat, especially nano- or microfat, creates soft transitions, better skin quality and a more natural overall impression. In addition, autologous fat has a regenerative effect and can last many years, sometimes permanently.
How soon is one socially presentable after the procedure?
As a rule our patients are socially presentable immediately – albeit with some limitation. Mild swelling or bruising is normal after the procedure but can be concealed well after just a few days. After about 10 – 14 days the unwanted side effects have usually subsided.
Is the result permanent or only visible short-term?
With the One-Stitch facelift we achieve a stable tightening, since the tissue is repositioned without tension. How long the tightening lasts depends on elasticity and skin thickness – this is about 3 years. The additionally introduced autologous fat can partly integrate permanently and improves skin quality over the long term. The natural ageing process is slowed, not abruptly stopped.
Can the eye area also be treated?
Through the tightening of the outer face there is a slight lifting of the eyebrows. This opens up the gaze and conveys a more youthful overall impression. In addition we can gently smooth and balance the eye region with nano- or microfat. Also suitable for dark circles.
What risks or side effects are to be expected?
The procedure is performed minimally invasively and on an outpatient basis at our practice in Düsseldorf. The risks are very low with specialist expertise. Typical accompanying effects are temporary swelling or bruising. As we work with the body's own fat, no long-term problems are to be expected.
When is a classic facelift more sensible than the extended One-Stitch facelift?
With pronounced skin laxity or significant tissue excess, a more extensive facelift is often the better option. The One-Stitch facelift is suitable above all for people who do not yet need major lifting but want more than a minimally invasive treatment.
What further combinations are sensible?
