Eyes: eyelids, under-eye bags, eyebrows, and more

Eyes: eyelids, under-eye bags, eyebrows, and more

The tissue around the eyes is thin and prone to wrinkles, excess connective tissue, and deposits of fat and tissue fluid. When firmness and elasticity diminish, this is where the signs appear first.

Eyelid lifts, the brow lift, and the treatment of under-eye bags and malar bags are a specialist discipline within plastic and aesthetic surgery. We have been treating patients for 30 years – using both classical and modern techniques, with virtually scar-free incisions, minimally invasive methods, or, depending on the concern, innovative radiofrequency approaches.

We advise you on corrections of the eyelids, eye area, and eyebrows – using proven and modern techniques. At artethic® in Düsseldorf and Berlin.

When is treatment advisable?

  • When hooded eyelids affect your field of vision or expression
  • For visible under-eye bags or puffiness
  • For volume loss beneath the eyes or at the sides of the brows
  • For a drooping brow and tired appearance
  • For persistent malar bags / festoons
  • For dark circles under the eyes
  • When the skin has become significantly lax

Upper eyelid lift (surgical)

In an upper eyelid lift we remove excess skin from the upper lid and, where necessary, any lax fatty or muscular tissue. The aim is a defined lid crease and a more open gaze. The treatment is performed on an outpatient basis, with the incision placed within the natural lid fold. Suitable for hooded eyelids, excess skin, or a tired appearance.

Lower eyelid lift (surgical)

Under-eye bags, wrinkles, and lax tissue are corrected. Depending on the findings, the skin may additionally be tightened, and the subcutaneous fatty tissue repositioned or reduced. The treatment improves the transition between the eyelid and the cheek. Suitable for age-related changes of the lower lid or visible under-eye bags.

Eyelid lift (combined)

Upper and lower eyelid correction can be performed in a single session if desired. We recommend this approach when both areas are affected or when aesthetic harmony across the entire eyelid region is the goal. The treatment follows the same principles as individual corrections. Individually planned and carried out with as little impact on the skin as possible.

Brow lift (surgical)

A surgical brow lift raises descended brow arches, creating more space at the upper lid and giving the gaze a more open appearance. Depending on the anatomy, the treatment is performed via small incisions at the hairline or on the forehead. Suitable for laterally drooping brows or significant skin tension on the upper lid.

Brow lifting (injectable)

The brow lift is a non-surgical method of elevating the eyebrows. Specific areas are treated with targeted injections, usually using a combination of hyaluronic acid and botulinum toxin.

  • Hyaluronic acid compensates for volume loss in the brow and forehead area and can selectively lift the tissue.
  • Botulinum toxin relaxes the fine muscles that pull the brow downward.

The result: a subtle lift and softer contours. Suitable for mild brow descent or as an alternative to surgery.

Injectable treatment for dark circles

Dark circles are often caused by loss of fatty tissue in the area between the lower lid and the cheek. With a subtle injectable treatment, we smooth the contour and reduce the shadowing effect. The eyes appear more alert and refreshed. Materials: hyaluronic acid or, depending on findings, fat transfer.

Not suitable for pronounced under-eye bags or significantly lax or loosened skin; other treatments are indicated in such cases. Also not suitable where there is a tendency to develop iatrogenic malar bags, which frequently arise as a result of filler injections. Precise expertise is essential here: knowing when to use which filler, when NCTF or polynucleotides or alternatively PRP are appropriate. We will advise you in detail and on an individual basis – at artethic® in Düsseldorf and Berlin.

Fat transfer for the eye area

Fat transfer is a natural and long-lasting way to restore volume loss in the eye area. The body's own fat is gently harvested, prepared, and precisely injected. It is particularly well suited for deep dark circles, hollow areas beneath the eyes, or to complement an eyelid lift.

The treatment is technically demanding and requires experience in plastic surgery. The result is harmonious and natural, as the body's own material is used.

Malar bags / festoons

Malar bags, also known as festoons, are swellings located between the lower eyelid region and the upper cheek. They differ from under-eye bags: they sit lower, towards the cheekbone, and consist of loose tissue, fluid, and fatty deposits.

Treatment is complex and depends on the extent of the condition. Options include surgical approaches, fat correction, and minimally invasive tightening procedures.

Which technique is right for which concern?

Frequently asked questions

Is aesthetic treatment of the eye area possible without surgery?

If your concerns – such as drooping skin, wrinkles, or under-eye bags – are mild, treatment is also possible without surgery. Examples include brow lifting (injectable), hyaluronic acid for dark circles, or volume restoration with fat transfer.

Of course, the effects are not as pronounced as with a surgical brow lift or eyelid lift – which today, incidentally, can be performed with minimal impact on the skin, virtually no visible scarring, and very little downtime.

What matters is choosing a qualified practitioner – specifically, specialist physicians in plastic and aesthetic surgery. And an obligation-free consultation prior to treatment.

Can minimally invasive and surgical techniques be combined?

Depending on the individual situation, this can absolutely be worthwhile. Combinations often achieve the best results – for example, a lower eyelid lift with additional volume restoration using fat transfer, or a subtle brow elevation. The most appropriate combination depends on your clinical findings and desired outcome.

How long do the results of the various eye and eyelid treatments last?

Surgical treatments such as upper and lower eyelid lifts or a brow lift typically last many years. Injections with polynucleotides, PRP, NCTF, or Quantum RF last several months – depending on skin quality and individual anatomy, up to a year or more. Fat transfer tends to last somewhat longer.

Why is the treatment of malar bags particularly complex?

Malar bags lie at the level of the cheekbone – more precisely, just below the eye area. They develop through a combination of fluid accumulation, fat, and tissue weakness. Malar bags are persistent and do not generally resolve on their own.

Sometimes radiofrequency treatments or minimally invasive methods are sufficient to achieve improvement – but surgical treatment is often required. A thorough analysis (distinguishing between iatrogenic malar bags caused by filler injections or other treatments, and those that are hereditary) and treatment tailored to the cause and current condition by a specialist physician in plastic and aesthetic surgery are strongly recommended.
