Treating dark circles and eye wrinkles with autologous fat


Autologous fat is a natural filler that can integrate into the tissue on a long-term basis. Its longevity is significantly greater than that of other fillers. As it consists of the body's own fat cells, the treatment (known as lipofilling) is exceptionally well tolerated. For more pronounced concerns, however, this approach has its limitations. An alternative is augmentation with autologous fat. Its key characteristics:

Dark circles develop when the tissue between the lower eyelid and the cheek loses volume. The contour appears more defined, light reflects unfavourably, and a "shadow effect" is created.


The treatment: we prepare the previously harvested autologous fat and inject it into the skin using fine cannulas. Hollows are filled in seamlessly, without visible transitions.

We use microfinely prepared fat. Large fat droplets are unsuitable and can create unevenness.


Treatment should be carried out in layers to avoid overcorrection.


In cases of under-eye bags or malar bags, autologous fat can be counterproductive and may worsen the problem. Alternatives are available.


Dark circles cannot be treated with botulinum toxin. This active ingredient is suited to smoothing crow's feet, relaxing the eyelid margin musculature, or softening certain areas of the outer eye. It is NOT applied directly to the lower eyelid, but to the lateral (outer) eye area.


Skin boosters (specialist, fluid hyaluronic acids) are not suitable for the lower eyelid region, but may be beneficial in the adjacent cheek area.

Results and longevity


A proportion of the transplanted fat cells integrate permanently. Another portion is reabsorbed over the first few weeks – this is normal. The final result becomes apparent after approximately two to three months.

Safety and tolerability


Autologous fat is the body's own substance – allergic reactions do not occur. That said, the eye region is anatomically demanding and sensitive. Directly after the treatment, and for up to 14 days, swelling and mild bruising may occur. These are normal responses to the treatment and will resolve on their own.

How is autologous fat harvested and prepared?


The fat is most commonly harvested from the abdomen or hips. It is then professionally cleansed and processed into very fine micro-droplets. We use specialist equipment for this purpose (such as a centrifuge). The fat is separated from tissue fluid and enriched with growth factors, which enhance the skin's natural regeneration and its "rejuvenation potential".

How long do the results of autologous fat treatment around the eyes last?


A portion of the fat integrates permanently into the tissue. Full stability is reached after approximately 2–3 months. Good to know: many of our patients report results that last considerably longer than fillers – often several years. Longevity also depends on individual metabolism and skin quality.

What happens if hollows are still visible after the first treatment?


As a proportion of the fat is reabsorbed, a second, minor session may be beneficial. We work deliberately conservatively, as overcorrection of the eye area can appear unnatural – the skin here is, after all, very thin. A gradual approach delivers the most convincing and natural-looking results.
